Submit the address information first and you will then be able to add the quantity of envelope imprints you desire to the shopping cart.  Of course, we've assumed that you've added (or will add) an equal or greater number of cards to the shopping cart! 

Please fill in the following form with the information as you want it to appear on the envelopes.  Carefully check for spelling errors and correct capitalization.  Cards by Anne is not responsible for errors made in the completion of this form.

Do not use the "Enter" key on your keyboard until all the information is added to the form.  Instead use your "Tab" key or your mouse to move from one textbox to the next.
